Most third party paying agencies require written justification to support the expenditure of higher priced equipment such as wheelchairs with dynamic tilt and power chairs. The agency may require that a form be completed or require an essay type evaluation. Although policies are different from agency to agency, if one follows the guidelines below this should be sufficient for most applications.

Essay presentations

Essay presentations should follow a S.E.T. procedure indicated by bold face type below. In most cases, medical need for the equipment must be shown. For example: the client needs the equipment to prevent a pressure sore, to facilitate respiration or prevent deformity. When the clinician's evaluation includes objective S.E. T. data, this provides the clinician and the third party paying agency with a firm foundation on which to make decisions. We recommend that essay type histories be written in the first person as this style of writing suggests that the author will take responsibility for the content.
